Birthday Party Reservation Form
Lifestyle Kids Club is very excited to announce a new program just for your children!
Lifestyle Kids Club Birthday Party Program.
In honor of your child's birthday, we invite you to host a birthday party at the Kid's Club. All birthday parties will be held on Sundays when the Kid's Club will be open for your child and their friends ONLY. The cost is $100 for up to 10 children and $5 for each additional child. 
Birthday parties will last 2 hours and include:
- 45 minutes of a Fit Kids class taught by a Lifestyle Team Member
- Choose from three themes:
- Fantasy Land
- Sports
- Balloon Bash
In addition, we've teamed up with several local businesses to make party planning easier on your wallet!
- Order your child's cake from Coldstone Creamery and receive free personalization and $3.00 off.
- Shop for party supplies and decorations at Party America and receive 10% off.
- No party is complete without pizza. Little Caesar's will also take 10% off your order!

Signing up is quick and easy! You have Two Options
- Fill out the registration form and ask any additional questions you would like to. Jesse will be contacting you to follow up. Then, click the Paypal deposit button to give us your $50 deposit.
- The registration form is available for PDF download. You can print it, fill it out and bring it in to us. Or fax it to 240-332-5818.
Simply download the registration form and reserve your spot. A $50.00 non-refundable deposit is required at the time of sign-up. We are looking forward to making your child’s birthday as magical as it should be.
